Often compared to a brick wall, the skin barrier is composed of interlocking pentagonal corneocytes (bricks) enmeshed in a lipid matrix (mortar). Abnormalities in the structure and function of the skin barrier underpin a number of dermatological conditions, the most common of which are atopic dermatitis/eczema and xerosis (dry skin). Being a highly visible part of our bodies, alterations in the structure of the skin barrier also change our appearance and affect our psychological wellbeing. We now need to try out this method on a new group of people. We are also interested in whether we can detect signs of inflammation in skin that looks and feels normal. Funded by: Sanofi Contact details: skinresearch@sheffield.ac.uk laura.johnson80@nhs.net rebecca.smith251@nhs.net Status: Closed to recruitment Please click on poster for more details This trial aims to answer the question that every parent asks about Dupilumab treatment for their child’s atopic dermatitis (atopic eczema). “How long will we have to use Dupilumab before it is safe to stop it and the atopic dermatitis (AD) will not return?” In order to have the best chance that AD will not return it is important to continue the Dupilumab until it has gone from both the surface of the skin and underneath the skin. We have developed a virtual skin biopsy using Optical Cohesive Tomography (OCT). This allows us to see through the skin using infra-red light to determine if the AD under the skin has resolved. OCT is a non-invasive imaging tool, capable of visualising beneath the skin surface to a depth of up to 2mm. Light is beamed into the skin, the precise timing of returning reflections are then measured and used to build up a map of the internal tissue structure. This beam is scanned laterally in order to form a three-dimensional volume scan.
